PRE-K PROGRAM
PRE- K REGISTRATION
AGE ELIGIBILITY:
- Date of birth must fall between October 1, 2021 and September 30, 2022.
PROCESS:
- Apply at ZONED school Tuesday - Friday 8:30 AM - 3:00 PM
REQUIRED DOCUMENTATION:
- Birth Certificate
- Social Security Card
- Up-to-date Immunization Record
- Proof of Residence (3 Items)
- Proof of Income (Household)
PROOF OF RESIDENCE (3 Items Needed):
One (1) of the following:
- Deed or property tax statement
- Mortgage statement
- Rental agreement/lease or notarized statement from landlord
Two (2) of the following:
- Utility bill
- Phone bill
- Cable bill
- Bank statement
- Insurance statement
- Current government issued document that contains the same address (i.e. SNAP card, W-2 form, passport, voter registration, driver’s license)
PROOF OF INCOME CAN INCLUDE:
- The last TWO (2) consecutive check stubs for EACH PARENT or CAREGIVER IN THE HOUSEHOLD for the current year
- SNAP/Food stamp award letter and the page that list those awarded including the parent’s name and the name of the child that is registering (SIS coordinator will verify in escholar)
- Current foster care placement agreement from DCFS
- An official letter from your employer stating place of employment, hourly rate of pay, and average number of hours worked per week
- Copy of EACH PARENT or Caregiver's W-2
- A statement from the Social Security Administration verifying that the child listed on the application receives SSI benefits
- LDOE Statement of No Income
- LDOE Declaration of Income for Irregular Employment
